Private GP Surgery is the trading name for Medical Health Limited (MHL). It is a private limited company registered in England and Wales under company number 12705438. It is providing its health care services under the umbrella of CQC registered health bodies which are GDPR compliant and registered with ICO.

Which information we collect:

  • Your demographics: (name, date of birth, gender, ethnicity)
  • Your contact details (email address, phone number and address)
  • Your emergency/Next of Kin contact details (optional)
  • Proof of Identity (e.g. Passport / Driving License)
  • You will also have the option of giving us your medical history (medical problems, medications, allergies, reason for booking) while filling the consent form. This could also be collected during consultation. Further detailed medical history will be taken during consultation.
  • Bank / Card details for payment purposes.

When do we collect this information:

We collect above information at time of booking an appointment (online, via phone/email, or at clinic).  We also collect the information during the consultation. We may also collect some information at the time of your initial inquiry via phone, email, contact form, online or whatsapp and any other platforms you choose to contact us. 

How do we store this information:

This information is stored securely using our GDPR compliant practice management system – Semble. We also use GDPR compliant email system. Some information may be stored in our emails, whatsapp, or other social medica platforms if it is collected to answer your inquiries. 

Why we collect this information:

Our primary goal of collecting this personal information is to provide you with healthcare services. This also allows us to communicate effectively with you. We may use this information to update you regarding services / products in relation to your health. We may also use this information for audit purposes in an anonymised fashion to improve our service. The information is also needed to collect payments for use of our services.

Who we may share this information with, in order to provide you healthcare services:

  • Anyone you ask us to share with or provide as an emergency contact, for example, your next of kin.
  • Members of support staff involved in the delivery of your care.
  • With another healthcare professional (e.g. doctor, nurse, carer etc) and their administration team involved in your treatment, or when making new referrals.
  • Diagnostic service providers e.g. laboratories and scanning services.
  • Third parties assisting in your healthcare such as insurance companies and courier companies.
  • Our indemnity provider and our solicitors in the event of a civil claim being brought against us.
  • Our third-party services providers such as IT suppliers, online pharmacies.
  • Payment providers.
  • A regulatory or public body, The Care Quality Commission, or the Court if we are duty bound to disclose or share your personal data in order to comply with a legal obligation, including a crime being investigated or a formal court order.
  • Our CQC registered partners.

We do not accept any responsibility or liability for third party service providers.

Disclosing your personal information:

Your data will never be provided to any third party without your consent, unless required by law. Occasionally, in extreme circumstances we may contact a third party (e.g. next of kin, social services, the DVLA, Police, your General Practitioner). This can happen if we feel there is a significant risk to you or others. We may also have to do this if there is a safeguarding concern or a situation where there is a concern about your mental capacity. However, even in these extreme circumstances we will try to obtain your consent first if possible and explain the reasons.

Limitations of Privacy Policy:

Please note our privacy policy does not cover your use of our social media platforms. We have no control on these. We do not accept any responsibility or liability for your sharing of your data on our social media platforms. Please refer to the privacy policies of individual social media platforms and whatsapp.

Access to information:

Medical Health Limited is registered with ICO.
