Mental Health Capacity Assessment Services

Mental Health Capacity Assessment

If you’re unsure which specific decision needs to be assessed, we can help you identify it and carry out a structured Mental Capacity Assessment that meets legal standards. This applies to a wide range of decisions, such as financial, property, legal, health and welfare.

COP3 Capacity Assessment

We carry out the full mental capacity assessment COP3 form for the Court of Protection using legally accepted language, so your solicitor can submit everything without delays or redrafting.

Lasting Power of Attorney

We check if the person fully understands what it means to give Power of Attorney for finance, property, health and welfare. If they do, our GP can also sign the official form as the Certificate Provider, so everything gets done in one appointment.

Power of Attorney to Manage Property and Finance

A power of attorney mental capacity assessment confirms that the person can understand and communicate financial choices. The Mental Health Act, Section 12 approved Private GP completes required mental capacity assessment form so solicitors or banks can act the same day.

Power of Attorney for Health and Welfare

We assess whether a person can make decisions about their treatment, where they want to live, and how they’re cared for each day. The report clearly explains how we are assessing mental capacity regarding people’s daily living choices.

Testamentary Capacity / Capacity to make Will

This mental capacity assessment form makes sure that the person fully understands what they’re doing when they make a will. This helps ensure the will is valid and less likely to be disputed in the future.

Litigation Capacity

We can also provide Litigation capacity assessments. Litigation capacity refers to a person’s legal ability to initiate or participate in legal proceedings.

Capacity Assessment for Equity Release

A capacity assessment for equity release determines whether a person has the mental capacity to understand and make informed decisions about entering into an equity release agreement—typically involving converting home equity into cash, often through a lifetime mortgage or home reversion plan.

Form PA14
Medical Certificate (Probate)

Probate is the legal right to deal with someone’s property, money and possessions (their ‘estate’) when they die. Sometimes the closest or legal person may lack capacity. Our doctor can assess if someone lacks capacity and complete the form PA14 if needed.
