At Private GP Surgery, we provide official medical certificates for individuals applying for an exemption from the UK Home Office’s English language requirement due to medical or psychological conditions.
KOLL stands for Knowledge of Language and Life in the UK. It is the Home Office requirement that most people must meet when applying for settlement or citizenship.
If you have a long-term physical or mental condition that prevents you from meeting this requirement, a qualified doctor can complete the official Home Office exemption form on your behalf.
Our role is the medical part of that process: assessing the condition and completing the form. The immigration application itself is submitted by you or your representative.
M.D - GMC Reg. 6106538
Private GP Surgery clinic is led by Dr. M. Ali Mahmood (GMC reg. 6106538), a General Practitioner with over 20 years in healthcare, a Doctor of Medicine (M.D.), full GMC registration with a licence to practise.
Dr.Ali will assess the condition and complete the official Home Office exemption form on your behalf.
The KOLL requirement usually applies at two stages of the immigration journey. The waiver can be relevant to both, where a long-term condition applies.
Applications for Indefinite Leave to Remain that require knowledge of language and life in the UK.
Applications to naturalise as a British citizen with the same requirement.
You may qualify if a long-term physical or mental health condition prevents you from taking the tests. Our doctors carry out a full assessment to confirm whether your condition may meet the Home Office criteria. Examples include general categories such as:
Long-term neurological conditions
Long-term mental health conditions
Long-term developmental conditions
Long-term cognitive conditions
The condition should be long-term that is, expected to last, or to have lasted, at least 12 months and its effect on your ability to take the tests is what the doctor assesses. If you are unsure whether your situation qualifies, speak to our team before booking.

Completed where you are eligible
An exemption is not guaranteed. We assess whether your condition may meet the Home Office’s criteria and, where you are eligible, complete the official KOLL waiver form based on our clinical assessment.
The final decision on any exemption rests solely with UK Visas and Immigration (UKVI). Where you already have medical reports setting out your diagnosis, we recommend submitting them alongside the form to support your application.
This service does not include immigration or legal advice. For guidance on your application, please refer to GOV.UK or a qualified immigration adviser.
Our team answers enquiries every day between 8AM and 9PM.
